3. Telehealth & Medical Records (Tebra Health)

   While you interact with the PelviZen website for initial discovery, your formal telehealth consultations and medical records are managed through Tebra Health, our secondary outside Electronic Medical Record (EMR) provider.

   Security Safeguards: PelviZen LLC utilizes Tebra’s HIPAA-compliant platform to ensure your health information is protected. We adopt Tebra’s robust technical and administrative safeguards as part of our commitment to you, including:

   • Encryption: Data is encrypted both in transit and at rest using FIPS 140-2 validated standards.
   • Access Control: Use of multi-factor authentication (MFA) and role-based access to ensure only authorized providers can view your data.
   • Monitoring: 24/7 threat detection, intrusion prevention, and regular security audits (SOC 2 Type 2).

4. Payments (Stripe)

   All financial transactions are processed through Stripe

   • Privacy Compliance: Your payment information is handled in accordance with Stripe’s Privacy Policy.
   • Data Isolation: To maintain your privacy, we do not store health-specific "diagnosis" data within Stripe. Stripe only receives the minimal information required to process your payment securely

   You have the right to access and request corrections to your data at any time.

   • Breach Notification: In the highly unlikely event of a data breach that compromises your personal information, PelviZen LLC will notify you promptly via the email address on file, in accordance with applicable legal requirements.
